Comparison of Therapeutic Efficacy of Combined Blood Expander Therapy and Combined Therapy of Diuretic Agent, and Antiviral Agent for Sudden Sensorineural Hearing Loss

Abstract
Background and Objectives: The idiopathic sudden sensorineural hearing loss is a disease that develops within several hours to several days. Its etiology is not verified yet, but the disturbance of blood circulation in the inner ear, intracochlear membrane breaks, inner ear hydrops and viral infection are considered as potential causes of disease. Therefore, no specific regimen is available for the treatment of this disease. The purpose of this study is to evaluate the therapeutic efficacy between the combined blood expander therapy and the combined therapy of diuretics and antiviral agent in sudden sensorineural hearing loss patients with many variables known as prognostic factors.

Materials and Methods: Clinical analysis was performed in 148 patients with sudden hearing loss admitted in GyeongSang National University from January 1994 to December 2002.148 ears were divided into the group A and group B. Blood expander was added to the therapeutic regimen in the group A. Combined diuretics and antiviral agents were added to the therapeutic regimens in the group B. The efficacy of the combined blood expander therapy and the combined therapy of diuretics and antiviral agent was analyzed according to the variables known as prognostic factors.

Results: The total recovery rate of the group B(51.9%) was not significantly different but higher than that of the group A(44.9%)(p=0.199). The therapeutic results of the group B were better than those of the group A on most prognostic factors. Especillay, statistical difference was found in the following two factors£»one associated with the vertigo symptom, and the other associated with the upper respiratory tract infection.

Conclusion: These results suggest that the combined therapy with diuretics and antiviral agents seems to be more effective than the com bined therapy with blood expanders to the recovery of sudden sensorin eural hearing loss in the cases of who have a history of vertigo and upper respiratory tract infection.
